As a practice our patients' views are important to us. We have set up a Patient Participation Group (PPG) to provide an opportunity for you to discuss with us how we can develop and shape our services to best meet our patients' needs.
To join our Patient Participation Group
We aim for the group to be as truly representative of our patient population as possible; all age groups are very welcome to join. If you would like to become a member, please complete the application form here: PPG Application Form You can print this out and hand it in at reception, or e-mail it to firstname.lastname@example.org
You will be contacted in due course. Providing the information asked for on the application form will help us to ensure that the PPG is as representative as possible of our practice population as a whole. The information you supply will be used lawfully, in accordance with the Data Protection Act 2018.
We hope you see this invitation as a positive opportunity to increase your understanding of how the surgery is run and to get involved with health care within your community.
