Community Practitioners

Our team of Community Practitioners triage and visit patients in their own homes or residential/nursing homes . They work closely with our GP and Nursing Team, along with the District Nursing Team to ensure those patients who are unable to come into surgery for an appointment receive the appropriate medical care. 

Lead by our Lead Nurse, Helen Kerrigan-Hawkes, this team currently comprises of two Advanced Nurse Practitioners, Donna Corrigan and Joanne Kerr, along with Pete Cooke our Paramedic.

 

First Contact Musculoskeletal (MSK) Practitioner

Fraser Snape is our First Contact Practitioner, Musculoskeletal Specialist who can see patients with Musculoskeletal presentations as the first point of contact. Patients are able to be seen at the earliest opportunity by a highly skilled practitioner in managing their problem, therefore reducing patient waiting time and the overall patient burden on GP/NP appointments.

 

Practice Pharmacists

Our team of Clinical Pharmacist Prescribers work alongside and support our GP and Nursing Team. They are able to provide a number of services for our patients, including clinical appointments for a range of medical issues. These currently include:

  • Medication queries
  • UTI – Urinary Tract Infections
  • Sore throats
  • Tonsillitis
  • Conjunctivitis
  • Sinusitis
  • Bites and stings

Another vital role they play is Medicine Management—this includes regular patient medication reviews ensuring medications are prescribed safely and used appropriately and clinical based decision making on the use of repeated requests for medication.
