We work closely alongside our colleagues within our Integrated Care Community. This includes our dedicated team of district nurses, community matron, care co-ordinators, health visitors and midwife. We also liaise very closely with the extended members of this team including mental health services, social services, local fire service and the police. We strive to ensure patients are looked after as far as possible within the community rather than having to stay in hospital. We hold monthly meetings with representatives from all these different services to ensure we are communicating effectively with each other to provide the best community care for our more vulnerable patients.
