Chronic Disease Management (CDM) Nurse

Band 6 | 18 Hours per Week

An exciting opportunity has arisen for an enthusiastic and motivated Chronic Disease Management (CDM) Nurse to join our large, friendly multidisciplinary team at our modern, purpose-built practice in the heart of Garstang.

We are seeking a qualified nurse who is passionate about delivering high-quality patient care and who can work both independently and collaboratively within a supportive team environment.

 

Main Duties

The successful candidate will undertake clinical reviews and provide holistic, evidence-based care in line with practice protocols for patients with:

  • Diabetes
  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD), including spirometry
  • Ischaemic Heart Disease (IHD)
  • Hypertension
  • Chronic Kidney Disease (CKD)

 

In addition, the role will include providing cover for Treatment Room duties such as:

  • Wound care
  • Vaccinations and immunisations, including childhood immunisations
  • Phlebotomy
  • ECGs
  • Blood pressure monitoring
  • NHS Health Checks

Experience in cervical cytology and pessary fitting would be advantageous.

Working Hours

18 hours per week

  • Tuesday: 8.00am to 6.00pm
  • Friday: 8.00am to 6.00pm
