Confidentiality
All the members of the Primary Health Care Team respect the confidentiality of patients and their medical records at all times. As patient records are held on computer, the Practice is registered with the Data Protection Registrar in accordance with the Data Protection Act 2018.
We are unable to disclose or discuss medical information with family members, friends or Carers without consent from the patient. If you wish to give permission for someone to discuss your medical record and treatment on your behalf, please complete a Consent form and return it to the surgery. This will then be documented to your medical record.
If a patient is unable to give consent, evidence of Lasting Power of Attorney for Health and Welfare will be needed.
